The National Visa Application Form is a primary document for those applying for the Digital Nomad Visa (DNV) via a Spanish Consulate. It is a standard multi-page form where the applicant provides personal details, passport data, and information about the purpose of stay. It is crucial to check the box for 'Residencia' and specify 'Ley 14/2013' or 'Teletrabajador de carácter internacional' in the purpose section.

No apostille or sworn translation is required for the form itself as it is a bilingual document provided by the Ministry of Foreign Affairs. It must be completed in Spanish or English (depending on the consulate's rules), printed, and signed by the applicant. One photo (passport size, white background) must be attached.

Common errors include providing an incorrect address in Spain (a hotel or temporary rental is usually sufficient), leaving mandatory fields blank, or choosing the wrong visa type. Regarding social security, if the applicant is a freelancer (autónomo) planning to register for 'alta en RETA', they will have access to the Spanish public healthcare system (SNS), so a private insurance policy might not be mandatory if the consulate accepts the social security commitment. However, requirements vary by consulate. The form is valid only for the specific appointment date.
